We use a secure website called Patient Access which provides:
- Appointments Booking
- Repeat Prescriptions
- Change of Address or Telephone Number
- Access parts of your GP medical record
Sign in to Patient Access.
However you will need to register for Patient Acces before you can use it.
The NHS App is the simple and secure way to access a range of NHS services (including services provided by Patient Access) on your smartphone or tablet.

How to Register for Patient Access
For security reasons you must prove your identity to the reception staff who will then supply a registration document with personalised access code.
You can only apply in person and we require one photographic identification (ID) (e.g. Passport or Driving Licence). This service is for patients over 16 years of age only.
To register visit the Patient Access registration website and follow the on screen instructions.
Alternatively you can download and complete a Patient Access Application Form and either return it to the surgery, or email to firstname.lastname@example.org
